1. Please download and fill out the following form below to order your Capital Beltway Warrior Uniform.
2. If you have not been assigned a jersey number, please check the Roster on TeamSnap, and find a number that is not currently taken, and select the number you would like.
3. If you do not have access to TeamSnap, please reach out to cbwarriors@cbwhockey.com so he can give access.
4. Once you have an available number, please confirm by email at cbwarriors@cbwhockey.com  

The price for a complete uniform order is $175.00 (with shipping) includes 2 jerseys (Home and Away), 2 pairs of socks (Home and Away), and 1 hockey pant shell.  You will be given information on how to pay once you have submitted your order.  

If you are buying replacement items, select the item you are purchasing, our supplier will still require the $5.00 per person shipping regardless of how many items you order. 

NOTE:  You must use the sizing charts on Page 2 before placing your order.
